I've been playing Transcendence for years, and there are still things/achievements I have to do, like permadeath

You should add a Bonus question 2: Did you ever finish a game without backtracking ?

Luckily you can do permadeath and no backtrack at the same time. That one game counts double then, right?

Not usually, but on the occasional good game I have.

Edit: After reading through the other answers, I should point out that I beat the old Heretic, when it was just the one enemy that you had to not destroy

My answer is for 1.01 alone. I still have to finish 1.05. I have a simple observation: Why is 1.01 much harder to play than 1.05? Is it just me or just me? Im confused now....
