George - it seems to be entirely random :/
I've had it twice on runs where I was testing weapon graphics with godmod (out of 6 test runs) and it has not happened (yet) with the single playthrough I'm doing.
And I have heard two accounts of it happening during normal playthroughs from what I can tell (no mods AFAIK), the one in the ticket, and someone on IRC.
Tomorrow night I'm taking the train home (3+ hr trip), and so I'll do some more runs & see if I can repro it.

Alright yeah, I tried to do this on several systems, and I could not reproduce it.

I tried a few games ago to get it on my screen camera, but my video software failed.
